LicenseLookup is operated by Synergetic Ventures LLC ("we" or "us"). By using the site you agree to these terms. If you don't agree, please don't use the site.

What this site is

LicenseLookup is an independent reference guide. It explains how licensing works and links to the official government sites where you can check a license yourself. It is general information only.

What this site isn't

• We are not a government agency, and we aren't affiliated with or endorsed by any licensing board.
• We don't issue, verify or certify licenses. The agency that issued a license holds the official record.
• We don't recommend, rate or endorse any person or business, and being listed or found here isn't an endorsement.
• Nothing here is legal, medical, financial, tax, insurance or engineering advice.
• Using the site doesn't create a doctor-patient, attorney-client or any other professional relationship.
• A statement that a state has no license for something describes what we found on the date shown. It isn't a legal opinion.

Health care pages

Our pages on doctors, nurses, dentists and optometrists contain links to state boards and general information. We keep no records about individual providers, and what you see on a board's site comes from that board. An active license doesn't measure quality of care or show that a provider is right for you. Talk to a qualified provider about your health. In an emergency, call 911.

Don't use this site for screening

LicenseLookup is not a consumer reporting agency, and the site doesn't provide consumer reports. Don't use anything here to decide someone's eligibility for employment, credit, insurance, housing or any other purpose covered by the Fair Credit Reporting Act. Licensing records can match the wrong person when names are similar, so confirm identity with the license number on the official record.

Accuracy

We work from official sources and record when we checked them, but rules change, agencies move their pages and public data can be wrong or out of date. License data we show comes from public agency files and may lag behind the agency's live record. The site is provided as is, without warranties of any kind. Always confirm a license and any rule with the agency before you act on it.

Links to other sites

We link to government and agency sites and to VetMyProposal, a separate tool from the same team. We don't control other sites and aren't responsible for their content, availability or accuracy.

Limits on liability

To the fullest extent the law allows, Synergetic Ventures LLC and the people behind it aren't liable for any loss or damage arising from your use of the site or your reliance on anything on it, or on any site we link to. That includes decisions to hire or not hire anyone.

Your responsibility

You decide who to hire or trust, and you're responsible for that decision. You agree to use the site lawfully and not to copy or scrape it in bulk. If your misuse of the site leads to a claim against us, you agree to cover the resulting costs.

Governing law

Florida law governs these terms, without regard to its conflict of law rules, and any dispute belongs in the state or federal courts in Florida. If a court finds part of these terms unenforceable, the rest still applies.

Corrections

If something here is wrong, see our corrections page or email hello@vetmyproposal.com. For a mistake in a license record itself, contact the agency that issued the license.

Changes

We may update or remove any part of the site or these terms at any time. The version on this page is the one that applies.
